Whereas, the claimant, Farmers Insurance on behalf of Maureen Conaty, C.I. File No. 1048-2020-437, has filed a claim against the City of Milwaukee in the amount of $22,259.76, and
Whereas, The City has agreed to settle the claim of Farmers Insurance on behalf of Maureen Conaty in the amount of $22,259.76, and
Whereas, the City Attorney recommends and the Common Council of the City of Milwaukee deems it expeditious and just to pay these claims; now, therefore, be it
Resolved, by the Common Council of the City of Milwaukee that proper city officers be and hereby are authorized and directed to issue a City check in the amount of $22,259.76, payable to the order of Farmers Insurance and Maureen Conaty, c/o Farmers Insurance, Duane Whitehead, Fire Insurance Exchange, Case Receipts Department, PO Box 268992, Oklahoma City, OK, 73126-8992, in full and final payment of these claims, upon receipt of releases of claims, approved as to form and execution by the City Attorney; said amounts to be charged to (DA) 636506-0410-6411-2631-R643.
