WHEREAS, the Housing Authority of the City of Milwaukee (Authority) entered into Contract 17-003 with KPH Environmental Corporation (Milwaukee, WI) for Hazardous Materials Abatement Services at Various Subsidized Public Housing Sites for an initial contract authority of $150,000.00 not-to-exceed; and
WHEREAS, the contract has been monitored for several months to ensure the long-term viability of continuing services under the contract; and
WHEREAS, the Authority successfully limited the volume of requested services within the available funding for the contracted services; and
WHEREAS, KPH Environmental Corporation has performed satisfactorily thus far on the contract; and
WHEREAS, the Authority desires to increase the contract authority by an additional $150,000.00 not-to-exceed for future services on an as-needed basis for the remainder of the initial contract period (ending March 27, 2018); therefore, be it
RESOLVED, by the Commissioners of the Housing Authority of the City of Milwaukee that they hereby authorize and approve execution of Change Order 1 to Contract 17-003 with KPH Environmental Corporation for Hazardous Materials Abatement Services at Various Subsidized Public Housing Sites in the amount of $150,000 not-to-exceed; and, be it
FURTHER RESOLVED, that the proper officers of the Authority be and hereby are authorized and directed to properly execute said contract change order for and on behalf of the Authority; and, be it
