This resolution further transfers the certificate for Ambulance Service Provider Area #2 from Meda-Care Ambulance Corp. to Paratech Ambulance Service, Inc.
Body
Whereas, Meda-Care Ambulance Corp. and Paratech Ambulance Service, Inc. are both currently certified by the Health Department in the manner specified in s. 75-15-8 of the Code; and
Whereas, Paratech Ambulance Service, Inc. has acquired Meda-Care Ambulance Corp.; and
Whereas, Paratech Ambulance Corp. has agreed, as specified in s. 75-15-13-b-1 of the Code, to take responsibility for Area #2, an area formerly serviced by Meda-Care Ambulance Corp., now, therefore, be it
Resolved, By the Common Council of the City of Milwaukee, that assignment and assumption of the private ambulance service provider agreement of Meda-Care Ambulance Corp. by Paratech Ambulance Service, Inc., a copy of which is attached to the file, is approved; and, be it
Further Resolved, That the certificate for Area #2, as stipulated in the private ambulance service provider agreement specified in s. 75-15-13-b-2 of the Code, is transferred from Meda-Care Ambulance Corp. to Paratech Ambulance Service, Inc; and, be it
Further Resolved, That the proper City officials are authorized to execute this agreement on behalf of the City of Milwaukee.
