Substitute resolution approving an agreement between the City of Milwaukee and Froedtert Health, Inc. for services related to the City's wellness program.
Analysis
This resolution approves an agreement between the City of Milwaukee and Froedtert Health, Inc. for services related to the City's wellness program. The Department of Employee Relation is directed to execute this agreement on behalf of the City. The resolution also directs that the outcomes-based component of the City's wellness program be resumed.
Body
Whereas, Common Council File Number 100204, adopted July 27, 2010, approved a one-year contract with an option of 2 additional years with Froedtert Health, Inc., to provide services to the City to implement its wellness program; and
Whereas, Common Council File Number 130115, adopted July 23, 2013, directed the Department of Employee Relations to initiate an outcome-based wellness program in which employees are eligible to potentially receive $150 per employee to be deposited into a Health Reserve Account; and
Whereas, Common Council File Number 131169, adopted December 17, 2013, directed the Department of Employee Relations to take the appropriate steps necessary to suspend the outcomes-based component of the City's wellness program; and
Whereas, The circumstances which led to the suspension of the outcomes-based component of the City's wellness program has resulted in a re-negotiation of the terms of the agreement between the City and Froedtert Health, Inc.; and
Whereas, The City Attorney's Office has negotiated a Restated Agreement with a one-year term and the option to extend for 2 one-year periods, a copy of which is attached to this file; and, now, therefore, be it
Resolved, By the Common Council of the City of Milwaukee, that the Restated Agreement between the City and Froedtert Health, Inc., regarding the provision of services by Froedtert Health, Inc., to the City for the City's wellness program is approved in substantially the same form as is attached to this file; and, be it
Further Resolved, That the Department of Employee Relations is directed to execute the agreement on behalf of the City; and, be it
Further Resolved, That the Department of Employee Relations is directed to resume the outcomes-based component of the City's wellness program.
