Substitute resolution relative to application, funding and expenditure of the 2009 Back to School Health Fair Grant from the Stark Hospital Foundation.
Analysis
This resolution authorizes the Health Department to apply for, accept and fund the 2009 Back to School Health Fair Grant from the Stark Hospital Foundation in the amount of $25,000 provided by the grantor. The purpose of the grant is to ensure health services and school supplies for school-readiness to children in need in Milwaukee.

Whereas, The City of Milwaukee appears to be eligible for grant funds from the Stark Hospital Foundation in this school-readiness initiative; and
Whereas, The operation of this grant project from 06/01/09 to 8/31/09 would cost $25,000 provided by the grantor; now, therefore, be it
Resolved, By the Common Council of the City of Milwaukee, that application to the Stark Hospital Foundation is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Milwaukee Code of Ordinances Section 304-81; and, be it
Further Resolved, That the City Comptroller is authorized to:
1. Commit funds within the Project/Grant Parent of the 2009 Special Revenue Grant and Aid Projects Funds, the following amount for the project titled 2009 Back to School Health Fair Grant from the Stark Hospital Foundation:

2. Create the necessary Special Revenue Fund - Grant and Aid Project/Grant and Project Grant Levels; budget against these Project/Grant values the amounts required under the grant agreement; and, be it
Further Resolved, That these funds are budgeted to the Health Department which is authorized to:
1. Expend from the amount budgeted sums for specified purposes as indicated in the grant budget and incur costs consistent with the award date;
2. Expend from the 2009 grant budget funds for training and out-of-town travel by departmental staff;
3. Enter into leases and subcontracts as detailed in the grant budget; and
4. Expend from the 2009 grant funds for equipment as deemed necessary for program operation.
