Substitute Resolution relative to application, acceptance and funding of the 2009 Milwaukee Intensive Nurse Home Visiting Program from Columbia St. Mary’s.
Analysis
This resolution authorizes the Health Department to apply for, accept and fund the Milwaukee Intensive Nurse Home Visiting Program from the Columbia St. Mary’s Hospital System in the amount of $86,997. The purpose of the project is to provide an intensive home visit based program to improve the health, well being and self sufficiency of at-risk low-income parents and their children.
Body:
Whereas, The City of Milwaukee appears to be eligible for grant funds from the Columbia St. Mary’s to provide this home visiting and health disparities program; and
Whereas, The operation of this grant project from 01/01/09 to 12/31/09 would cost $86,997 entirely provided by the grantor; now, therefore, be it
Resolved, By the Common Council of the City of Milwaukee, that application to the Columbia St. Mary’s is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Milwaukee Code of Ordinances Section 304-81; and, be it
Further Resolved, That the City Comptroller is authorized to:
1. Commit funds within the Project/Grant Parent of the 2009 Special Revenue-Grant and Aid Projects Fund, the following amount for the project titled Milwaukee Intensive Nurse Home Visiting Program:

2. Create the necessary Special Revenue Fund - Grant and Aid Project/Grant and Project/Grant Levels; budget against these Project/Grant values the amounts required under the grant agreement; and be it
Further Resolved, That these funds are budgeted to the Health Department which is authorized to:
1. Expend from the amount budgeted for specified purposes as indicated in the grant budget and incur costs consistent with the award date;
2. Enter into subcontracts and leases as detailed in the grant budget; and
3. Expend from the 2009 grant budget funds for training and out-of-town travel by departmental staff.
Further resolved, That the Common Council directs that the 2009 Positions Ordinance C.C. File Number 080522, should be amended as follows:
