Title
Substitute resolution authorizing the Department of Employee Relations (DER) to prepare and execute contracts with dental providers for a two year period with a one year option for pre-paid dental services and for fee-for-service dental services.

Whereas Common Council file 091535 authorized the Department of Employee Relations to complete a Request for Proposal process for a vendor for both pre paid dental plans and for a fee-for-service dental plan, and,
Whereas the Department of Employee Relations has completed the request for proposal for pre-paid dental plans, and,
Whereas there were only two vendors bidding on the pre paid dental services, and,
Whereas the City desires additional choices for a pre paid dental plan, and,
Whereas the current vendors Care Plus and Anthem Dental Blue have submitted bids for 2011-2012, and,
Whereas the City desires only one vendor for the fee-for-service dental plan, and
Whereas there were five respondents to the Request for Proposal, and,
Whereas Delta Dental, the current vendor, was the lowest cost vendor for 2012-2013, and,
Whereas the City pays a flat $13 for single dental and $37.50 for family dental and the employee pays the balance of the cost,
Therefore be it resolved that the Department of Employee Relations with the approval of the City Attorney’s office is authorized to enter into contracts with Care Plus Dental and Anthem Dental Blue for pre paid dental services and is authorized to enter into a contract with Delta Dental for a fee-for-service dental plan for a two year period, January 11, 2011 through December 31, 2012 with an option year.
