Analysis
This resolution authorizes the Health Department to accept and fund the FoodShare Outreach - Wisconsin Supplemental Nutrition Assistance Program (SNAP) Grant from the State of Wisconsin Department of Health Services in the amount of $245,270
with a grantor share of $122,635 (50%) and city match of $122,635 (50%). The purpose of this funding is to continue support for the City of Milwaukee Health Department (MHD) Community Healthcare Access Program (CHAP), formerly the Medical Assistance Outreach Program. Currently with CHAP there is a heavy focus on Medical/Healthcare Access servicing but with DHS funding this will bolster the program to improve access to healthy, nutritious food by increasing information about FoodShare and support applying for FoodShare benefits.

BODY:
Whereas, The City of Milwaukee appears to be eligible for grant funds from the State of Wisconsin Department of Health Services to continue support for the City of Milwaukee Health Department (MHD) Community Healthcare Access Program (CHAP) to improve access to healthy, nutritious food by increasing information about FoodShare and support applying for FoodShare benefits; and

Whereas, The operation of this grant program from 11.01.2021 to 09.31.2022 would cost $245,270 with a grantor share of $122,635 (50%) and city match of $122,635 (50%); now, therefore, be it

Resolved, By the Common Council of the City of Milwaukee, that application to the State of Wisconsin Department of Health Services is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Milwaukee Code of Ordinances Section 304-81; and, be it
