Title

Substitute resolution relative to acceptance and funding of the 2021-2022 Preventive Health Grant from the State of Wisconsin Department of Health Services.

 

Analysis

This resolution authorizes the Health Department to accept and fund the 2021-2022 Preventive Health Grant from the Wisconsin Department of Health Services in the amount of $57,836, 100% grantor share with no city match.  The purpose of the contract is to provide funding to develop a Community Health Improvement Plan (CHIP) which is a prerequisite for the accreditation process.

 

Body

Whereas, The City of Milwaukee appears to be eligible for grant funds from the State of Wisconsin Department of Health Services for the 2021-2022 Preventive Health Grant to develop the Community Health Improvement Plan for the accreditation process; and

 

Whereas, The operation of this grant project from 10/01/21 to 09/30/22 would cost $57,836 which would be provided entirely by the grantor; now, therefore, be it

 

Resolved, By the Common Council of the City of Milwaukee, that application to the Wisconsin Department of Health and Family Services is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Milwaukee Code of Ordinances Section 304-81; and, be it

 

Further Resolved, That the City Comptroller is authorized to:

 

1.  Commit funds within the Project/Grant Parent of the 2021 Special Revenue-Grant and Aid Projects Fund, the following amounts for the project titled 2021-2022 Preventive Health Grant:

2.  Create the necessary Special Revenue Fund - Grant and Aid Project/Grant and Project/Grant Levels; budget against these Project/Grant values the amount required under the grant agreement;

 

Further Resolved, That these funds are budgeted to the Health Department which is authorized to:

 

1.  Expend from the amount budgeted sums for specified purposes as indicated in the grant budget and incur costs consistent with the award date;

 

2.  Expend from the 2021 grant budget funds for specific items of equipment;

 

3.  Expend from the 2021 grant budget funds for training and out-of-town travel by departmental staff;

 

4.  Enter into subcontracts as detailed in the grant budget; and, be it

 

Further Resolved, That the Common Council directs that the 2020 Positions Ordinance C.C. File Number 200537, should be amended as follows:
