This resolution further transfers the certificate for Ambulance Service Provider Area #1 from Paratech Ambulance Services, Inc. to Midwest Medical Transport Company, LLC.

Whereas, Paratech Ambulance Service, Inc. and Midwest Medical Transport Company, LLC are both currently certified by the Health Department in the manner specified in s. 75-15-8 of the Code; and
Whereas, Midwest Medical Transport Company has acquired Paratech Ambulance Service, Inc.; and
Whereas, Midwest Medical Transport Company has agreed, as specified in s. 75-15-13-b-1 of the Code, to take responsibility for Area #1, an area formerly serviced by Paratech Ambulance Services, Inc.; now, therefore, be it
Resolved, By the Common Council of the City of Milwaukee, that assignment and assumption of the private ambulance service provider agreement of Paratech Ambulance Service, Inc. by Midwest Medical Transport Company, LLC, a copy of which is attached to the file, is approved; and, be it
Further Resolved, That the certificate for Area #1, as stipulated in the private ambulance service agreement specified in s. 75-15-13-b-2 of the Code, is transferred from Paratech Ambulance Services, Inc. to Midwest Medical Transport Company, LLC; and, be it
Further Resolved, That the proper City officials are authorize to execute this agreement on behalf of the City of Milwaukee.
