Resolution approving Amendment No. 2 to Contract No. 15-015 Professional Service Industries, Inc. (Waukesha, WI) for Professional Services to Provide Hazardous Materials and Environmental Hazards Management Consulting Services at Various Sites
HACM is requesting to exercise its option to renew the contract for one year, the second and last renewal option pursuant to the terms of the contract. The current contract authority is $500,000 not to exceed, expiring 12/31/2017. The amendment is for a time extension only, to 12/31/2018. No additional contract authority dollars are required at this time.

WHEREAS, the Housing Authority of the City of Milwaukee (Authority) by adoption of Resolution No. 12725, on November 11, 2015, authorized and approved the entering into of a contract between Professional Service Industries, Inc. (PSI) and the Authority for hazardous materials and environmental hazards management consulting services at various sites; and
WHEREAS, Professional Service Industries, Inc. has performed the services satisfactorily during the first and second years, and it is in the best interests of the Authority to extend their contract at the same terms and conditions of the original contract;
WHEREAS, PSI has agreed to renew the Contract at the same rates, terms, and conditions as the original contract;
WHEREAS, PSI triggered Section 3 with a need to subcontract and has subcontracted 34% with a Section 3 and MBE business. As a result of subcontracting in excess of 25% to a Section 3 Business Concern, PSI is a Section 3 Business and the entire dollar amount of the contract is counted towards Section 3; now, therefore, be it
RESOLVED, by the Commissioners of the Housing Authority of the City of Milwaukee that they hereby authorize and approve amending Contract No. 15-015 by extending the contract through December 31, 2018 with no increase in the total contract price; and, be it
FURTHER RESOLVED, that all other terms and conditions of the subject contract, not hereby amended, shall remain unmodified and in full force and effect; and, be it
FURTHER RESOLVED, that the proper officers of the Authority be and hereby are authorized and directed to execute, for and on behalf of the Authority, Amendment No. 2 to Contract No. 15-015 to effectuate the intent and purpose of this Resolution.
