Substitute resolution relative to the acceptance and funding of the 2017 Immunization Action Plan Grant from the State of Wisconsin Department of Health Services.

Analysis
This resolution authorizes the Health Department to accept and fund the 2017 Immunization Action Plan Grant from the Wisconsin Department of Health Services in the amount of $262,221. The purpose of the contract is to increase the City immunization compliance rates of children to 92% by the end of 2017.

Whereas, The City of Milwaukee appears to be eligible for grant funds from the State of Wisconsin Department of Health Services for the 2017 Immunization Action Plan Grant to increase immunization rates; and

Whereas, The operation of this grant project from 01/01/17 to 12/31/17 would cost $262,221 would be provided entirely by the grantor and; now, therefore, be it

Resolved, By the Common Council of the City of Milwaukee, that application to the Wisconsin Department of Health and Family Services is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Milwaukee Code of Ordinances Section 304-81; and, be it

Further Resolved, That the City Comptroller is authorized to:

1. Commit funds within the Project/Grant Parent of the 2017 Special Revenue-Grant and Aid Projects Fund, the following amounts for the project titled 2017 Immunization Action Plan Grant:
