Title
Substitute resolution relative to the acceptance and funding of the Adult Immunization Grant from the Wisconsin Department of Health Services, Division of Public Health.
Analysis
This resolution authorizes the Health Department to accept and fund the Adult Immunization Grant from the Wisconsin Department of Health Services, Division of Public Health in the amount of $56,833 provided by the grantor. The purpose of the grant is to increase adult immunization rates within the City of Milwaukee.

Whereas, The City of Milwaukee appears to be eligible for grant funds from the Wisconsin Department of Health Services, Division of Public Health for adult immunization; and
Whereas, The operation of this grant project from 05/01/16 to 06/30/17 would cost $56,833 provided by the grantor; now, therefore, be it
Resolved, By the Common Council of the City of Milwaukee, that application to the Wisconsin Department of Health Services, Division of Public Health is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Milwaukee Code of Ordinances Section 304-81; and, be it
Further Resolved, That the City Comptroller is authorized to:
1. Commit funds within the Project/Grant Parent of the 2016 Special Revenue Grant and Aid Projects Funds, the following amount for the project titled Adult Immunization Grant:

2. Create the necessary Special Revenue Fund - Grant and Aid Project/Grant and Project Grant Levels; budget against these Project/Grant values the amounts required under the grant agreement; and, be it
Further Resolved, That these funds are budgeted to the Health Department which is authorized to:
1. Expend from the amount budgeted sums for specified purposes as indicated in the grant budget and incur costs consistent with the award date;
2. Expend from the 2016 grant budget funds for training and out-of-town travel by departmental staff;
3. Enter into leases and subcontracts as detailed in the grant budget; and
4. Expend from the 2016 grant funds for equipment as deemed necessary for program operation.
Further resolved, That the Common Council directs that the 2016 Positions Ordinance, C.C. File 150505, should be amended as follows:

To expire 06/30/17 unless the Adult Immunization Grant from the Wisconsin Department of Health Services, Division of Public Health is extended. Partially funds one Public Health Nurse Coordinator, and one Program Assistant in the Immunizations Program.
