Title

Substitute resolution relative to continued funding of a Milwaukee Intensive Nurse Home Visiting Program from Columbia St. Mary’s.

 

Analysis

This resolution authorizes the continued funding of the Milwaukee Intensive Nurse Home Visiting - Columbia St. Mary’s grant.

 

Body

Whereas, Common Council File Number 081098, adopted April 14, 2009, authorized application for, acceptance and funding for the Milwaukee Intensive Nurse Home Visiting Grant through funding from Columbia St. Mary’s; and

 

Whereas, The project, as originally funded by Columbia St. Marys, had two components; the Nurse Family Partnership and the Center for Health Equity

 

Whereas, The Center for Health Equity portion of the project has an unencumbered balance at the end of 2015 of $41,284 which has been advanced by Columbia St. Mary’s; and

 

Whereas, Milwaukee Code of ordinances Section 304.81 states that the “carrying over the unexpended funds shall be limited to three years from the year in which the funds were appropriated. A grant project exceeding this period shall be re-funded through a separate resolution”; and

 

Whereas this limitation applies to the continuation of the Milwaukee Intensive Nurse Home Visiting - Columbia St. Mary’s grant, including the portion of this grant related to the Center for Health Equity; and

 

Whereas, Continued involvement in this program will allow the Center for Health Equity to continue to address the Social Determinants of Health, which are known to be the strongest drivers of health outcomes and health disparities,

 

Now, therefore, be it

 

Resolved, By the Common Council of the City of Milwaukee, that continuation of the grant for Milwaukee Intensive Nurse Home Visiting - Columbia St. Mary’s, to be administered by the Health Department, is hereby authorized; and, be it

 

Further Resolved, That the proper City officials and/or the herein named department head are authorized to continue the grant without further Common Council approval; and be it

 

Further Resolved, That the Health Department is authorized to expend, from the amount appropriated, sums for specified purposes in accordance with the previously approved ongoing budget for the program: and be it

 

Further Resolved, That the Health Department shall have the authority to continue transfers within aforementioned project budget as long as the amounts expended are in accordance with Grantor regulations.
