Body

Whereas, The City of Milwaukee, the American Pharmacist Association (APhA) Foundation and local sponsors, including Julie Whipple of “Use Your Medications Wisely” have agreed to offer a self-management diabetes program and services to City of Milwaukee employees, retirees and their spouses; and

Whereas, The City of Milwaukee desires to provide incentives to those persons who participate in the self-management program by meeting with pharmacists on a regular basis to review their diabetes control efforts; Now, therefore, be it

Resolved, That The City of Milwaukee agrees to provide a one-time $100 adjustment on the Basic Plan monthly premium to those persons who participate for at least six months in the Ten City Diabetes Challenge.
