Resolution relative to application, acceptance and funding of the Glasses for Kids Grant. (Health Department)

 

Analysis

This resolution authorizes the Health Department to apply for, accept and fund the Glasses for Kids Grant from the United Way of Greater Milwaukee in the amount of $30,000. The purpose of the project is to provide vision screening, eye examinations and vision vouchers for eyeglasses for the Milwaukee school age population who do not have access to eye care and lack health insurance. 

 

Body:

Whereas, The City of Milwaukee appears to be eligible for grant funds from United Way of Greater Milwaukee to provide vision screening, eye examinations and vision vouchers for eyeglasses for the Milwaukee school age population who do not have access to eye care and lack health insurance; and

 

Whereas, The operation of this grant project from 03/01/03 to 12/31/05 would cost $30,000 entirely provided by the grantor; now, therefore, be it

 

Resolved, By the Common Council of the City of Milwaukee, that application to United Way of Greater Milwaukee is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 030712 (Grant Ordinance); and, be it

 

Further Resolved, That the City Comptroller is authorized to:

 

1.  Commit funds within the Project/Grant Parent of the 2004 Special Revenue-Grant and Aid Projects Fund, the following amount for the project titled Glasses for Kids Grant:

2.  Create the necessary Special Revenue Fund - Grant and Aid Project/Grant and Project/Grant Levels; budget against these Project/Grant values the amounts required under the grant agreement; and, be it

 

Further Resolved, That these funds are budgeted to the Health Department which is authorized to:

 

1.  Expend from the amount budgeted for specified purposes as indicated in the grant budget and incur costs consistent with the award date;

 

2.  Enter into subcontracts and leases as detailed in the grant budget.
