Title
Resolution relative to application, acceptance and funding of the Federally Qualified Health Center Grant.
 
Analysis
This resolution authorizes the Health Department to apply for, accept and fund the Federally Qualified Health Center Grant project from the Human Resources and Services Administration of the U.S. Department of Health and Human Services in the amount of $1,082,500 provided by the grantor. The purpose of the program is to serve the medically underserved citizens and at-risk community members of the City of Milwaukee.  
 
Body
Whereas, The City of Milwaukee appears to be eligible for grant funds from the Human Resources and Services Administration of the U.S. Department of Health and Human Services to serve the medically underserved citizens and at-risk community members of the City of Milwaukee; and
 
Whereas, The operation of this grant project from 09/16/03 to 09/15/04 would cost $1,082,500 provided by the grantor; now, therefore, be it
 
Resolved, By the Common Council of the City of Milwaukee, that application to the State of Wisconsin Division of Health and Family Services is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 030712 (Grant Ordinance); and, be it
 
Further Resolved, That the City Comptroller is authorized to:
 
1.  Commit funds within the Project/Grant Parent of the 2004 Special Revenue Grant and Aid Projects Fund, the following amount for the project titled Federally Qualified Health Center Grant:

2.      Create the necessary Special Revenue Fund - Grant and Aid Project/Grant and Project/Grant Levels; budget against these Project/Grant values the amount required under the grant agreement.

Further Resolved, That these funds are budgeted to the Health Department which is authorized to:
                  
1.  Expend from the amount budgeted sums for specified purposes as indicated in the grant budget and incur costs consistent with the award date;
 
2.   Enter into subcontracts and leases as detailed in the grant budget;   
 
3.      Expend from the 2004 grant budget funds for training and out-of-town travel by departmental staff; and
 
4.      Expend from the 2004 grant budget funds for specific items of equipment.
