Title
Substitute resolution relating to accepting rental and service agreements with BACCORP Development Ltd. to provide optometric care services at the Isaac Coggs Community Health Center and the Johnston Community Health Center.
Analysis
This resolution authorizes service and rental agreements between the City of Milwaukee and BACCORP Development Ltd., which will occupy space at 2 City of Milwaukee health facilities, the Isaac Coggs Community Health Center and Johnston Community Health Center, and will provide optometric
care services at these centers as part of the Municipal Health Services Program.
Body
Whereas, A service agreement has been negotiated between the City of Milwaukee Health Department and BACCORP Development Ltd., in which BACCORP Development Ltd. has agreed to provide through December 31, 2004, optometric care services to uninsured and underinsured elderly clients as part of the federally-funded Municipal Health Services Program; and

Whereas, A rental agreement has been negotiated between the City of Milwaukee Health Department and BACCORP Development Ltd., in which BACCORP Development Ltd. will be provided space at the Isaac Coggs Community Health Center, 2770 North 5th Street, and the Johnston Community Health Center, 1230 West Grant Street, and shall make quarterly payment of rent to the City of Milwaukee; and

Whereas, On behalf of the U.S. Department of Health Centers for Medicare and Medicaid Services, the City of Milwaukee Health Department shall administer the service and rental agreements for BACCORP Development Ltd.'s programs at the 2 health centers; and

Whereas, BACCORP Development Ltd. will be providing services formerly provided by Sight Center Inc., which discontinued optometric care services in November 2002; now, therefore, be it
