Resolution relative to application, acceptance and funding of an Immunization Action Plan Grant. (Health Department)
ANLS:
This resolution authorizes the Health Department to apply for, accept and fund an Immunization Action Plan Grant from the Centers for Disease Control through the Wisconsin Department of Health and Family Services in the amount of $173,985. The purpose of the project is to raise the age-appropriate immunization levels of two-year old children in Milwaukee County.
BODY:
Whereas, The City of Milwaukee appears to be eligible for grant funds from the Centers for Disease Control through the Wisconsin Department of Health and Family Services for raising the age-appropriate immunization levels of two-year old children in Milwaukee County; and

Whereas, The operation of this grant project from 01/01/99 to 12/31/99 would cost $173,985 provided by the grantor; now, therefore, be it

Resolved, By the Common Council of the City of Milwaukee, that application to the Wisconsin Department of Health and Family Services is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 940843 (Grant Ordinance); and, be it

Further Resolved, That the City Comptroller is authorized to commit funds within the Project/Grant Parent 1999 Special Revenue Grant and Aid Project fund, the following amount for the project titled Immunization Action Plan Grant:
