Analysis
This resolution authorizes the Health Department to apply for, accept and fund the Milwaukee Intensive Nurse Home Visiting Program from Columbia St. Mary’s in the amount of $500,000. The purpose of the project is to provide an intensive home visit based program to improve the health, well being and self sufficiency of at-risk low-income parents and their children as well as to address health disparities in Milwaukee with the creation of the MHD Center for Health Equity.
Body:
Whereas, The City of Milwaukee appears to be eligible for grant funds from the Columbia St. Mary’s to provide this home visiting and health disparities program; and

Whereas, The operation of this grant project from 01/01/07 to 12/31/07 would cost $500,000 entirely provided by the grantor; now, therefore, be it

Resolved, By the Common Council of the City of Milwaukee, that application to the Columbia St. Mary’s is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 030712 (Grant Ordinance); and, be it

Further Resolved, That the City Comptroller is authorized to:

1. Commit funds within the Project/Grant Parent of the 2006 Special Revenue-Grant and Aid Projects Fund, the following amount for the project titled Milwaukee Intensive Nurse Home Visiting Program and MHD Center for Health Equity:
