Substitute resolution approving an amendment to the group life insurance policy
Analysis
Section 350-25-4-b-4 of the Milwaukee City Code authorizes employees represented by certain unions to apply for supplemental coverage in $1,000 increments to a maximum of $100,000 coverage and authorizes employees at their own expense to purchase life insurance in excess of 1½ times their base salary. Under the terms of the contract with the carrier, such change in coverage requires the approval of the carrier. The carrier has established underwriting requirements and rates for providing the coverage. This resolution approves the amendment to the group life insurance policy.

Whereas; Section 350-25-4-b-4 of the Code provides for voluntary supplemental life coverage to employees represented by certain unions; and
Whereas; The extension of such voluntary coverage at the expense of the employee requires an amendment to the contract between the city and United Wisconsin Group; and
Whereas; Section 350-24-4-b-4 of the Milwaukee Code states that the effective date of coverage following the open enrollment period is the first day of January of the following year; now, therefore, be it
Resolved; By the Common Council that the proposed amendment annexed to the file be and hereby is approved and the proper city officers be and hereby are authorized and directed to execute same on behalf of the city of Milwaukee.
