Substitute resolution relative to application, acceptance and funding of a Milwaukee Breast Cancer Awareness Program - Site Coordination Grant.
ANLS:
This resolution authorizes the Health Department to apply for, accept and fund a Milwaukee Breast Cancer Awareness Program - Site Coordination Grant from the State of Wisconsin, Division of Health and Family Services in the amount of $181,145 with a city share of $44,145 and a grantor share of $137,000. The purpose of the project is to increase the quality and scope of mobile mammography services.
BODY:
Whereas, The City of Milwaukee appears to be eligible for grant funds from the State of Wisconsin, Division of Health and Family Services for increasing the quality and scope of mobile mammography services; and
Whereas, The operation of this grant project from 10/01/98 to 09/30/99 (CFDA #93.399) would cost $181,145 of which $44,145 (24%) would be provided by the city and $137,000 (76%) would be provided by the grantor; now, therefore, be it
Resolved, By the Common Council of the City of Milwaukee, that application to the State of Wisconsin, Division of Health and Social Services is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 940843 (Grant Ordinance); and, be it
Further Resolved, That the City Comptroller is authorized to:
1. Encumber in the parent accounts of the 1998 Special Revenue Accounts-Grant and Aid Projects, the following amounts for the program titled Milwaukee Breast Cancer Awareness Program - Site Coordination Grant:
