Substitute resolution relative to application, acceptance and funding of the ASSIST Grant.
ANLS:
This resolution authorizes the Health Department to apply for, accept and fund the ASSIST Grant from the State of Wisconsin Division of Health and Family Services in the amount of $67,117 with a city share of $21,673 and a grantor share of $45,444. The purpose of the project is to support local and statewide community coalitions to reduce tobacco use in Wisconsin through action focusing on policy and media advocacy.
BODY:
Whereas, The City of Milwaukee appears to be eligible for grant funds from the State of Wisconsin Division of Health and Family Services for supporting local and statewide community coalitions to reduce tobacco use in Wisconsin through action focusing on policy and media advocacy; and
Whereas, The operation of this grant project from 10/01/97 to 09/30/98 would cost $67,117 of which $21,673(32%) would be provided by the city and $45,444(68%) would be provided by the grantor; now, therefore, be it
Resolved, By the Common Council of the City of Milwaukee, that application to the State of Wisconsin Division of Health and Family Services is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 940843 (Grant Ordinance); and, be it
Further Resolved, That the City Comptroller is authorized to:
1. Encumber in the parent accounts of the 1997 Special Revenue Accounts-Grant and Aid Projects, the following amounts for the project titled ASSIST Grant:
