Analysis

This resolution approves a contract between the City of Milwaukee and American Pharmacists Association Foundation to provide services to City of Milwaukee employees and retirees and their dependents enrolled in the Basic Plan health insurance through the Ten City Diabetes Challenge in 2006.

Body

Whereas, The City of Milwaukee desires to make sure employees, retirees and their dependents are taking control of their diabetes; and

 

Whereas, The American Pharmacists Association Foundation has invited the City to participate in their Ten City Diabetes Challenge Program, modeled after the successful program in Ashville, North Carolina; and

 

Whereas, The American Pharmacists Association Foundation and their members are providing funding for one year to cover costs associated with pharmacists providing educational services and counseling; and

 

Whereas, The American Pharmacists Association Foundation spoke to the Finance and Personnel Committee at their October 12, 2005 meeting regarding the services and benefits of the program; now, therefore, be it

 

Resolved, By the Common Council of the City of Milwaukee, that the appropriate City officials are authorized to sign the contract attached to this file between the American Pharmacists Association Foundation and the City of Milwaukee related to the Ten City Diabetes Challenge for a one-year period, January 1, 2006 through December 31, 2006.
