Resolution relative to application, acceptance and funding of the SURVNET Grant. (Health Dept.)
ANLS:
This resolution authorizes the Health Department to apply for, accept and fund the SURVNET Grant project from the Centers for Disease Control through the Wisconsin Division of Health and Family Services in the amount of $42,757 with a city share of $9,757 and a grantor share of $33,000. The purpose of the project is to create a county-wide system for reporting communicable diseases in Milwaukee.
BODY:
Whereas, The City of Milwaukee appears to be eligible for grant funds from the Wisconsin Division of Health and Family Services to establish a county-wide system for reporting communicable diseases; and

Whereas, The operation of this grant project from 10/01/97 to 09/30/98 would cost $42,757 of which $9,757 (23%) would be provided by the city and $33,000 (77%) would be provided by the grantor; now, therefore, be it

Resolved, By the Common Council of the City of Milwaukee, that application to the Wisconsin Division of Health and Family Services is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 940843 (Grant Ordinance); and, be it

Further Resolved, That the City Comptroller is authorized to:

1. Encumber in the parent accounts of the 1997 Special Revenue Accounts-Grant and Aid Projects, the following amounts for the project titled SURVNET Grant:
