Title
Substitute resolution relative to acceptance and funding of the 2023 Forward Health Outreach grant from the WI Department of Health Services.

Analysis
This resolution authorizes the Health Department to accept, and fund the 2023 Forward Health Outreach grant from the Wisconsin Department of Health Services in the amount of $646,721 with a City share of $323,361 and a grantor share of $323,361. The funding will provide 50% of the cost of the Health Department’s work to ensure MHD clients and City residents have access and assistance in enrolling and sustaining enrollment in public or private health insurance.

Body
Whereas, The City of Milwaukee appears to be eligible for grant funds from the Wisconsin Department of Health Services to assist City residents in enrolling and sustaining enrollment in health insurance through the Community Health Access Program (CHAP); and

Whereas, The operation of this grant program from January 1, 2023 to December 31, 2023 will cost $646,721 comprised of $323,361, or 50%, grantor share and $323,631, or 50%, City share; now, therefore, be it;

Resolved, By the Common Council of the City of Milwaukee, that the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Milwaukee Code of Ordinances Section 304-81; and, be it

Further Resolved, that the Commissioner of Health or their designee is authorized to execute the grant agreement, and associated data sharing agreements, for these funds; and, be it

Further Resolved, That the City Comptroller is authorized to:

1. Commit funds within the Project/Grant parent account of the 2023 Special Revenue-Grant and Aid Projects Fund, the following amounts for the project titled MA Assistance - CHAP:
