Title
Resolution relating to acceptance and funding of the 2026 Forward Health Outreach grant from the WI Department of Health Services.
Analysis
Resolution authorizes the Health Department to accept, and fund the 2026 Forward Health Outreach grant from the Wisconsin Department of Health Services in the amount of $1,123,238 with a City share of $561,619 and a grantor share of $561,619. The funding will provide 50% of the cost of the Health Department’s work to ensure MHD clients and City residents have access and assistance in enrolling and sustaining enrollment in public or private health insurance.
Body
Whereas, The City of Milwaukee appears to be eligible for grant funds from the Wisconsin Department of Health Services to assist City residents in enrolling and sustaining enrollment in health insurance through the Community Health Access Program (CHAP); and
Whereas, The operation of this grant program from January 1, 2026 to June 30, 2027 will cost $1,123,238 comprised of $561,619, or 50%, grantor share and $561,619 or 50%, City share; now, therefore, be it;
Resolved, By the Common Council of the City of Milwaukee, that the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Milwaukee Code of Ordinances Section 304-81; and, be it
Further Resolved, that the Commissioner of Health or their designee is authorized to execute the grant agreement, and associated data sharing agreements, for these funds; and, be it
Further Resolved, That the City Comptroller is authorized to:
1. Commit funds within the Project/Grant parent account of the 2026 Special Revenue-Grant and Aid Projects Fund, the following amounts for the project titled MA Assistance - CHAP:

2. Create the necessary Special Revenue Fund Grant and Aid Project/Grant and Project Levels; budget to these Project/Grant values the amounts required under the grant agreement; and
3. Establish the necessary City Share Project Values; and, be it
Further Resolved, That these funds are budgeted to the Health Department which is authorized to:
1. Expend from the amount appropriated sums for specified purposes as indicated in the grant budget and incur costs consistent with the award date;
2. Expend from the 2026 grant budget funds for specific items of equipment; and,
3. Expend from the 2026 grant budget funds for training and out-of-town travel by departmental staff; and,
4. Enter into subawards and leases as detailed in the grant budget.
