Resolution amending Common Council File 971017 relative to amending the application, acceptance and funding of the 1998 Prevention Block Grant.
ANLS:
This resolution amends Common Council File 971017 relative to amending the 1998 Prevention Block Grant. The purpose of the grant is to assure access to cardiovascular program services through a coordinated community effort.
BODY:
Whereas, Common Council File 971017 authorized the Health Department to apply for, accept and fund the 1998 Prevention Block Grant from the State of Wisconsin Division of Health and Family Services for the purpose of assuring access to cardiovascular program services through a coordinated community effort. This grant provided for a Grantor share total of $112,007 and a City share total of $34,849; and
Whereas, The Grantor share for this program remains unchanged; the in-kind City share is increased by $1,770; and the total cost is increased by $1,770; now, therefore, be it
Resolved, By the Common Council of the City of Milwaukee, that application to the State of Wisconsin Division of Health and Family Services is authorized and the Health Department shall accept such a grant without further Common Council approval unless the terms of the grant change as indicated in Common Council File 940843 (Grant Ordinance); and, be it
Further Resolved, That the City Comptroller is authorized to encumber in the parent accounts of the 1998 Special Revenue Accounts-Grant and Aid Projects, the following amounts for the program titled 1998 Prevention Block Grant:

Further Resolved, That these funds are appropriated to the Health Department which is authorized to:
1. Expend from the 1998 grant budget funds for specified items of equipment;
2. Expend from the 1998 grant budget funds for training and out-of-town travel by departmental staff;
3. Enter into subcontracts and leases as detailed in the grant budget;
and, be it
Further Resolved, That the Common Council directs that the 1998 Positions Ordinance, Common Council File 971100, should be amended as follows:
HEALTH DEPARTMENT PUBLIC HEALTH SERVICES DECISION UNIT Cardiovascular Risk Reduction Grant
Add:
(1) Health Project Coordinator (X) (2) Graduate Intern (Part-time, temporary)
Delete:
(1) Public Health Nurse Supervisor (X) (1) Public Health Educator (x)
Amend footnote (I) as follows:
To expire 09/30/98 unless the 1998 Prevention Block Grant, available from the State of Wisconsin Division of Health and Family Services, is extended. Positions are authorized only as reflected in the grant agreement as approved by the Grantor Agency; and, be it
Further Resolved, That all standard resolved clauses articulated in Common Council File 971017 remain applicable.
