Resolution relative to application, acceptance and funding of a Hepatitis B Immunization Grant. (Health Department)
ANLS:
This resolution authorizes the Health Department to apply for, accept and fund a Hepatitis B Immunization Grant project from the State of Wisconsin Division of Health and Family Services in the amount of $44,067 with a city share of $10,206 and a grantor share of $33,861. The purpose of the project is to initiate and complete the Hepatitis B vaccine series in high-risk infants.
BODY:
Whereas, The City of Milwaukee appears to be eligible for grant funds from the State of Wisconsin Division of Health and Family Services for initiating and completing the Hepatitis B vaccine series in high-risk infants; and
 
Whereas, The operation of this grant project from 01/01/98 to 12/31/98 would cost $44,067 of which $10,206 (23%) would be provided by the city and $33,861 (77%) would be provided by the grantor; now, therefore, be it
 
Resolved, By the Common Council of the City of Milwaukee, that application to the State of Wisconsin Division of Health and Family Services is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 940843 (Grant Ordinance); and, be it
 
Further Resolved, That the City Comptroller is authorized to:
 
1. Encumber in the parent accounts of the 1998 Special Revenue Accounts-Grant and Aid Projects, the following amounts for the project titled Hepatitis B immunization Grant:

2. Create a Special Revenue Fund - Grant and Aid Projects and the necessary expenditure and revenue accounts; appropriate to these accounts the amount required under the grant agreement and transfer the corresponding offsetting estimated revenue;
 
3. Establish the necessary grant performance submeasures; and, be it
 
Further Resolved, That these funds are appropriated to the Health Department which is authorized to:
 
Expend from the amount appropriated sums for specified purposes as indicated in the grant budget and incur costs consistent with the award date.
