Resolution relative to application, acceptance and funding of a Breast Cancer Awareness Grant. (Health Department)
ANLS:
This resolution authorizes the Health Department to apply for, accept and fund a Breast Cancer Awareness Grant program from the Wisconsin Division of Health and Social Services in the amount of $115,200. The purpose of the program is to provide outreach health care services related to mammography screening.
BODY:
Whereas, The City of Milwaukee appears to be eligible for grant funds from the Wisconsin Division of Health and Family Services for outreach health care services related to mammography screening; and
 
Whereas, The operation of this grant program from 01/01/00 to 12/31/00 (CFDA #435.119) would cost $115,200 provided by the grantor; now, therefore, be it
 
Resolved, By the Common Council of the City of Milwaukee, that application to the Wisconsin Division of Health and Family Services is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 940843 (Grant Ordinance); and, be it
 
Further Resolved, That the City Comptroller is authorized to:
 
1. Commit funds within the Project/Grant parent accounts of the 2000 Special Revenue Accounts-Grant and Aid Projects, the following amounts for the program titled Breast Cancer Awareness Grant:

2. Create the necessary Special Revenue Fund - Grant and Aid Project/Grant and Project/Level Values; budget to these Project/Grant values the amount required under the grant agreement;
 
3. Establish the necessary project values; and, be it
 
Further Resolved, That these funds are appropriated to the Health Department which is authorized to:
 
1. Expend from the amount appropriated sums for specified purposes as indicated in the grant budget and incur costs consistent with the award date; and
 
2. Expend from the 2000 grant budget funds for training and out-of-town travel by departmental staff.
