Analysis

This resolution authorizes the Health Department to apply for, accept and fund the MBCAP Outreach Grant project from the Susan G. Komen Foundation in the amount of $30,000 entirely provided by the grantor. The purpose of the project is to reduce mortality from breast cancer by increasing the number of low-income and under-served women 50 years of age and older.

Whereas, The City of Milwaukee appears to be eligible for grant funds from the Susan G. Komen Foundation to reduce mortality from breast cancer by increasing the number of low-income and under-served women 50 years of age and older; and

 

Whereas, The operation of this grant project from 01/01/04 to 12/31/04 would cost $30,000 entirely provided by the grantor; now, therefore, be it

 

Resolved, By the Common Council of the City of Milwaukee, that application to the Susan G. Komen Foundation is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 030712 (Grant Ordinance); and, be it

 

Further Resolved, That the City Comptroller is authorized to:

 

1.  Commit funds within the Project/Grant Parent of the 2004 Special Revenue-Grant and Aid Projects Funds, the following amount for the project titled MBCAP - Outreach Grant:

2.                     Create the necessary Special Revenue Fund - Grant and Aid Project/Grant and Project/Grant Levels; budget against these Project/Grant values the amount required under the grant agreement

Further Resolved, That these funds are budgeted to the Health Department which is authorized to:

                                                               

1.  Expend from the amount budgeted for specified purposes as indicated in the grant budget and incur costs consistent with the award date;

 

2.  Expend from the 2004 grant budget funds for items of equipment as may be deemed necessary for the effective operation of the program;

 

3.  Expend from the 2004 grant budget funds for training and out-of-town travel by departmental staff; and

 

4.  Expend from the 2004 grant budget funds for contracts and leases as required by the program.
