Resolution relative to acceptance and funding of the GIS Infant Mortality Study Grant. (Health Department)
ANLS:
This resolution authorizes the Health Department to accept and fund the GIS Infant Mortality Study Grant project from the Perinatal Foundation in the amount of $15,683 with a city share of $683 and a grantor share of $15,000. The purpose of the project is to link records of births and deaths with social, demographic and environmental data and analysis and mapping of factors that contribute to infant mortality in the City.
BODY:
Whereas, The City of Milwaukee appears to be eligible for grant funds from the Perinatal Foundation for linking records of births and deaths with social, demographic and environmental data and analysis and mapping of factors that contribute to infant mortality in the City; and
Whereas, The operation of this grant project from May 1, 1998 to April 30, 1999 would cost $15,683 of which $683 (4%) would be provided by the city and $15,000 (96%) would be provided by the grantor; now, therefore, be it
Resolved, By the Common Council of the City of Milwaukee, that application to the Perinatal Foundation has been authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 940843 (Grant Ordinance); and, be it
Further Resolved, That the City Comptroller is authorized to:
1. Encumber in the parent accounts of the 1998 Special Revenue Accounts-Grant and Aid Projects, the following amounts for the project titled GIS Infant Mortality Study Grant:

2. Create a Special Revenue Fund - Grant and Aid Projects and the necessary expenditure and revenue accounts; appropriate to these accounts the amount required under the grant agreement and transfer the corresponding offsetting estimated revenue;
3. Establish the necessary grant performance submeasures; and, be it
Further Resolved, That these funds are appropriated to the Health Department which is authorized to:
1. Expend from the amount appropriated sums for specified purposes as indicated in the grant budget and incur costs consistent with the award date;
2. Expend from the 1998 grant budget funds for training and out-of-town travel by departmental staff;
and
3. Enter into subcontracts and leases as detailed in the grant budget.
