Title

Resolution relative to application, acceptance and funding of the Asthma Care Coordination Grant.

 

Analysis

This resolution authorizes the Health Department to apply, accept and fund the Asthma Care Coordination Grant project from the State of Wisconsin Department of Health and Family Services through the Children’s Health Systems in the amount of $7,016. The purpose of the project will be to support care coordination and case management for children who are diagnosed with persistent and uncontrolled asthma.    

Body

Whereas, The City of Milwaukee appears to be eligible for grant funds from the State of Wisconsin Department of Health and Family Services through the Children’s Health Systems to support care coordination and case management for children who are diagnosed with persistent and uncontrolled asthma; and

 

Whereas, The operation of this grant project from 10/01/05 to 09/30/06 would cost $7,016 provided entirely by the grantor; now, therefore, be it

 

Resolved, By the Common Council of the City of Milwaukee, that application to the Children’s Health System is authorized  and the  Health Department shall accept this grant without further Common Council approval unless the terms of the grant change as indicated in Common Council File 030712 (Grant Ordinance); and, be it

 

Further Resolved, That the City Comptroller is authorized to:

 

1.  Commit funds within the Project/Grant Parent of the 2005 Special Revenue -Grant and Aid Projects Fund, the following amounts for the project titled Asthma Care Coordination Grant:

2.  Create the necessary Special Revenue Fund - Grant and Aid Project/Grant and Project/Grant Levels; budget against these Project/Grant values the amount required under the grant agreement; and, be it

 

Further Resolved, That these funds are budgeted to the Health Department which is authorized to:

 

Expend from the amount budgeted sums for specified purposes as indicated in the grant budget and incur costs consistent with the award date.
