Substitute resolution relative to application, acceptance and funding of the Making Connections Grant.
Analysis
This resolution authorizes the Health Department to apply for, accept and fund the Making Connections Grant project from Making Connections - Milwaukee in the amount of $20,000 grantor share. The purpose of the project is to assist low income families in obtaining medical insurance and food share services.
Body
Whereas, The City of Milwaukee appears to be eligible for grant funds from Making Connections - Milwaukee to assist low income families in obtaining medical insurance and food share services; and
Whereas, The operation of this grant project from 01/01/05 to 12/31/05 would cost $20,000 entirely provided by the grantor; now, therefore, be it
Resolved, By the Common Council of the City of Milwaukee, that application to Making Connections - Milwaukee is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 030712 (Grant Ordinance); and, be it
Further Resolved, That the City Comptroller is authorized to:
1. Commit funds within the Project/Grant Parent of the 2005 Special Revenue-Grant and Aid Projects Fund, the following amounts for the program titled Making Connections Grant:

2. Create the necessary Special Revenue Fund - Grant and Aid Project/Grant and Project/Grant Levels; budget against these Project/Grant values the amount required under the grant agreement; and, be it
Further Resolved, That these funds are budgeted to the Health Department which is authorized to:
Expend from the amount appropriated sums for specified purposes as indicated in the grant budget and incur costs consistent with the award date; and, be it
Further Resolved, That the Common Council directs that the 2005 Positions Ordinance C.C. File Number 040459, should be amended as follows:

Public Health Educator II (R)(X)
And under

Add:

Amend footnote (R) as follows:
