This resolution amends common Council File 031566 relative to application , acceptance and funding of the Fight Asthma Milwaukee Allies Grant project from the Children's Hospital of Wisconsin. The purpose of the project will be to support care coordination and case management for children who receive emergency department services due to the severity of their asthma.

Whereas, Common Council File 031566 authorized the Health Department to apply for, accept and fund the Fight Asthma Milwaukee Allies Grant from Children's Hospital of Wisconsin to support care coordination and case management for children who receive emergency department services due to the severity of their asthma. This grant provided for a grantor share of $73,975 and City Share of $24,412; and
Whereas, The Grantor Share for this program is increased by $53,045; now, therefore, be it
Resolved, By the Common Council of the City of Milwaukee, that the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 030712 (Grant Ordinance); and, be it
Further Resolved, That the City Comptroller is authorized to:
1. Commit funds within the Project/Grant Parent of the 2004 Special Revenue -Grant and Aid Projects Fund, the following amounts for the project titled Fight Asthma Milwaukee Allies Grant:

2. Create the necessary Special Revenue Fund - Grant and Aid Project/Grant and Project/Grant Levels; budget against these Project/Grant values the amount required under the grant agreement;

Further Resolved, That these funds are budgeted to the Health Department which is authorized to:
1. Expend from the amount budgeted sums for specified purposes as indicated in the grant budget and incur costs consistent with the award date;
2. Enter into contracts as indicated in the budget; and, be it
Further Resolved, That all standard resolved clauses articulated in Common Council File 031566 remain applicable.
