Title
Resolution relative to the acceptance and funding of the 2019 Immunization Action Plan Grant from the State of Wisconsin Department of Health Services.
Analysis
This resolution authorizes the Health Department to accept and fund the 2019 Immunization Action Plan Grant from the Wisconsin Department of Health Services in the amount of $243,667. The purpose of the contract is to increase the City immunization compliance rates of children to 93% by the end of 2019.

Whereas, The City of Milwaukee appears to be eligible for grant funds from the State of Wisconsin Department of Health Services for the 2019 Immunization Action Plan Grant to increase immunization rates; and
Whereas, The operation of this grant project from 01/01/19 to 12/31/19 would cost $243,667 would be provided entirely by the grantor and; now, therefore, be it
Resolved, By the Common Council of the City of Milwaukee, that application to the Wisconsin Department of Health and Family Services is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Milwaukee Code of Ordinances Section 304-81; and, be it
Further Resolved, That the City Comptroller is authorized to:
1. Commit funds within the Project/Grant Parent of the 2019 Special Revenue-Grant and Aid Projects Fund, the following amounts for the project titled 2018 Immunization Action Plan Grant:

2. Create the necessary Special Revenue Fund - Grant and Aid Project/Grant and Project/Grant Levels; budget against these Project/Grant values the amount required under the grant agreement;
3. Establish the necessary City Share Project Values; and, be it
Further Resolved, That these funds are budgeted to the Health Department which is authorized to:
1. Expend from the amount budgeted sums for specified purposes as indicated in the grant budget and incur costs consistent with the award date;
2. Expend from the 2019 grant budget funds for specific items of equipment;
3. Expend from the 2019 grant budget funds for training and out-of-town travel by departmental staff;
4. Enter into subcontracts as detailed in the grant budget; and, be it
Further Resolved, That the Common Council directs that the 2019 Positions Ordinance, C.C. File 180629, should be amended as follows:
