Substitute resolution relative to application, acceptance and funding of a Breast and Cervical Cancer Screening Grant.
ANLS:
This resolution authorizes the Health Department to apply for, accept and fund a Breast and Cervical Cancer Grant project from the State of Wisconsin Division of Health and Family Services in the amount of $180,181 with a city share of $43,181 and a grantor share of $137,000. The purpose of the project is to increase the number of low-income and under-served women 50 years of age and older to be screened for breast and cervical cancer and improve the quality of screening, referral and follow-up.
BODY:
Whereas, The City of Milwaukee appears to be eligible for grant funds from the State of Wisconsin Division of Health and Family Services for breast and cervical cancer screening, referrals and follow-ups for low-income and under-served women 50 years of age and older; and
 
Whereas, The operation of this grant project from 10/01/98 to 09/30/99 (CFDA #93.399) would cost $180,181 of which $43,181 (24%) would be provided by the city and $137,000 (76%) would be provided by the grantor; now, therefore, be it
 
Resolved, By the Common Council of the City of Milwaukee, that application to the State of Wisconsin Division of Health and Family Services is authorized and the Health Department shall accept this grant without further approval unless the terms of the grant change as indicated in Common Council File 940843 (Grant Ordinance); and, be it
 
Further Resolved, That the City Comptroller is authorized to:
 
1. Encumber in the parent accounts of the 1998 Special Revenue Accounts-Grant and Aid Projects, the following amounts for the program titled Breast and Cervical Cancer Screening Grant:

2. Create a Special Revenue Fund - Grant and Aid Projects and the necessary expenditure and revenue accounts; appropriate to these accounts the amount required under the grant agreement and transfer the corresponding offsetting estimated revenue;
 
3. Establish the necessary grant performance submeasures; and, be it
 
Further Resolved, That these funds are appropriated to the Health Department which is authorized to:
 
1. Expend from the amount appropriated sums for specified purposes as indicated in the grant budget and incur costs consistent with the award date;
 
2. Expend from the 1998 grant budget funds for training and out-of-State travel; and
 
3. Enter into subcontracts and leases as detailed in the grant budget; and, be it
 
Further Resolved, That the Common Council directs that the 1998 Positions Ordinance, C.C. File 970831, should be amended as follows:
